A MINIMUM OF 7 TOUCH POINTS OCCUR WITHIN THE FIRST TWO WEEKS OF DISCHARGE.
Week 1
- Pre-discharge visit at the hospital with the home health liaison and In-patient introduction phone call.
24-48 hours prior to discharge
- Home Visit #1 Includes medication reconciliation, safety check, assessment & education.
Day after discharge
- Tuck-in Phone Call #1 Identify red flags & schedule next home visit
1st Friday patient is at home
- Home Visit #2 Includes med compliance, vitals, assesment, and scheduling next home visit
1st weekend patient is at home
Week 2
- Home Visit #3 Includes medication compliance, vitals, and well-being assessment
Monday-Thursday Minimum of 1 home visit
- Tuck-in Phone Call #1 Address questions & schedule next home visit
2st Friday patient is at home
- Home Visit #4 Identify red flags & schedule next home visit
2nd weekend patient is at home
- Additional Home Health Visits as needed
